Starting with the 2019-2020 School Year, the Madison Central School District will be requiring ANNUAL/YEARLY physicals for 7th – 12th graders (current 6th – 11th graders) who participate in sports and/or marching band. Annual physicals can rule out injury or underlying health conditions that may have developed since the student previous exam. These annual exams will help to ensure that athletes are healthy and fit to participate in a safe manner.
Madison Regional Health Systems will be offering physicals at the school on the following dates, at a cost of $35:
- May 13th – High School Students (morning and afternoon)
- May 14th – High School Student (morning) & 7th Grade Students (afternoon)
- May 16th – Middle School students (morning and afternoon)
Mitch Poppens (Physicians Assistant) will be here to complete the physicals for the male students and Meggan Reisch (Nurse Practitioner) will be here to complete the physicals for the female students.
If you are interested in having your child get a physical, please call the school office (high school: 256-7706 or middle school: 256-7717) to reserve a spot. Then, print off and fill out the “Pre-Physical History Form” that’s attached and have your student return it to their school office – along with a check for $35 to “Madison Regional Health Systems” – and we will confirm their spot on the schedule.
**If you are unable to print from home, these forms will be available in both the high school and middle school offices.**
**Only physicals performed by a medical professional after April 1, 2019 are valid for the 2019-2020 school year.**
